The Institute for Medicaid Innovation created this toolkit as the first of a suite of resources to allow Medicaid managed care organizations (MCOs) to articulate the business case to launch, lead, and/or expand initiatives focused on NMDOH/SDOH. It is meant to increase social determinants of health (SDOH) investments by Medicaid health plans, state Medicaid agencies, health care, and community-based organizations (CBOs). This first phase, funded by the Episcopal Health Foundation, offers tools to begin authentic partnerships with CBOs and relationship management strategies.
