Using Risk Stratification to Identify and Address NMDOH Needs in Rural Communities
February 20 @ 11:00 am – 12:00 pm
Burke is the Local Mental Health Authority (LMHA) for eastern Texas, managing a census with a high rate of complex health needs. This session will outline the risk stratification process developed to enhance service delivery within two rural, outpatient primary care clinics. This approach prioritizes precision in identifying risk levels and needs, including NMDOH, to allocate limited resources and staffing effectively.
