Practical Steps to Integrating NMDOH Screening and Referral Program

Dr. Stanley Williams will share his experience integrating an NMDOH screening and resource referral through the Health Home program at the Harris Center for Mental Health and IDD. He will describe how they navigated the new screening process into program staff workflow, connected patients with resources based on need, and evaluated health outcomes to demonstrate the NMDOH program impact.
